Cats are very prone to stones and obstructions because of the lack of moisture in their diet. One of my boys had similar symptoms to what you described, and he also was diagnosed with a UTI. The antibiotics they gave him improved symptoms for a few days, but then I happened to notice him still straining, and blood in his urine. I took him to a different place, they did an ultrasound, and found stones. It might still be worth it to go get a second opinion.

I see you've bought a fountain, that's good. Running water encourages them to drink. Maybe, if the fountain didn't work, you can try leaving a faucet trickling, and see if he'll go for that? I would also suggest continuing wet food in his diet. I even add a little extra water for good measure. He has been on C/D for over six months now without a reoccurrence, do feed him both wet and dry. So far so good!

Anyway, you sound like you're doing everything right, but just be careful. I'm sure you know cats are masters of disguise.
