Oh my! So, he has scar tissue, already? I realize that may not be the only reason, but it is the usual reason. I wish I could help. And, I hope that other members have some experience with this and see this post soon. Personally, I am going to go search the internet, as this is driving me crazy.

If you don't get some responses soon, you might start a new post and give some basic background to help readers out. Sorry, but once a post gets so long/old, it tends to get less attention.

It was being severely structured, yes. Which INFURIATES ME because I brought him to 3 different other vets sine surgery non stop and everyone said he was not strictured...

And yes I will make another post, thank you.

Just an update... We have decided to go forth and do the surgery. There is no way we can live with ourselves knowing there was a chance he would survive. I don't know how we will go forward financially, but that is something I will just need to figure out.... The vet is going to begin it shortly, and will call part way through If they are unable to complete it and it just won't work. She said there is chance that will happen, but we just need to try...

Hey everyone, the vet just called. She said the surgery went as well as it could have.. She now has him catheterized and will keep him like that at the hospital for tonight and tomorrow night, hopefully he can come home Wednesday evening without needing to be catheterized at home. I'm so glad he made it... Now to the stress of hoping he doesn't re-stricture as apparently the odds are a lot higher after the revision surgery. :( Thanks for all your well wishes and love, it has helped so much.

PJ is home! He came home and immediately drank tons of water independently (he hadn’t drank any water out of bowl when he came home from vet last PU), and then ate some food. A decent amount too! He was given three drugs: buprenorphine (he was given 0.4mL every 12-24 hours depending on how he feels) for 7 more doses (3 were already given). I can request more if he seems to need it she said. Then he was given clavamox (1.5 mL every 12 hours), and robenacoxib (1 tab every 24 hours). He seems pretty pooped, and so happy to be home with us. About an hour into coming home.. he peed. And guess what. IT WASN'T DRIPS! IT WAS A STREAM! Happy day!! He then pooped a bit later as well, although very watery he did poop.

Just wanted to ask the group, anyone who has experience with these meds - any side effects I should be aware of? I know they said the pain meds he was given had quite a “drunk effect” on him but was wondering who else had experience with these. He’s only had clavamox in the past so far.

How is PJ doing? I just read through this whole thread as I’m having the same issues with my cat Yuri who had the PU surgery on November 15th. I’m scared that if we do the revision surgery this could happen all over again and the recovery from the 1st surgery was brutal but I also don’t want to constantly worry that he’s going to block again if we do nothing.

We were told that we can do the revision surgery or we can leave it and he could possibly be fine. We’re going to a different vet for a 2nd opinion this week. I'm so sorry to hear you're dealing with these issues as well. :( When you say you're experiencing the same thing, do you mean that Yuri is just dribbling urine rather than a full stream? Has he been doing this since day 1 of recovery, or did that just start? Did they say WHY Yuri is recommended to have a revision surgery? (Has he strictured??)

First off, I would definitely go to a 2nd vet for another opinion.. And if possible, if you have somewhere near you that has board certified specialists/surgeons to see, get a referral and go there. I went to 3 vets before finding out the real issue with PJ, which the specialists figured out. I spent SO MUCH extra money on all those extra vet visits, when going to the expensive specialist initially would have been cheaper.

The reason PJ had to get the revision was because he strictured, although I whole heartedly believe the first surgeon did it incorrectly - as he obviously didn't stricture the minute he was out of surgery, and he was never peeing properly once getting it, so it must have been something else at first. But, PJ is doing fantastic now compared to how he did the first time around. He is peeing properly (he pees about 2-6 times a day, lately on the lower end) with normal streams. No more straining, or back and forth constantly into the litter box. He unfortunately has had constant diarrhea since surgery, which is our concern right now. It's not good as he loses a lot of fluid this way, and he's lost a lot of weight. His urethra area seems to be a bit red and irritated as well, so we're taking him back into the specialist for her to see him shortly. But besides those issues, he is peeing which is most important and is so amazing to see. Of course I don't know all the details of Yuri's recovery so I don't want to recommend one thing or the other, but I'm curious as to why your vet wants him to get the revision? Generally cats only get told they need the revision surgery if it's absolutely necessary - or if it seems very unlikely your pet will recover otherwise. For PJ, it was life or death. If he didn't get the surgery he would slowly stricture more and more, and would continue suffering. So we were deciding between putting him down or getting the surgery. So, I'd like to know why Yuri was told to get it and then maybe I can help you out with some more details from PJ's experience. :)

Thanks for the reply! Glad PJ is urinating well. I feel like my life revolves around watching my cat use the litter box. Since the surgery Yuri only pees small amounts. We went back weekly after the surgery and everything looked fine apparently until the last appointment when the stitches were coming out the vet said she was concerned the hole looked very small and we should keep an eye on it. We’ve been back many times because he will occasionally start frequenting the litter box more often and then we’ll see blood but we’ve never really gotten answers about that just various medications.
On Thursday we brought him in and the plan was to sedate him, shave him and see what the hole looks like and hopefully make a small incision to make it bigger but I got a call that she couldn’t see the hole at all. She would squeeze his bladder and urine would come out but you couldn’t see where it was coming from. She said we could get a board certified surgeon to perform the PPU surgery or we could opt not to, she did not say it was life or death, she didn’t even mention the hole closing up more. When we went to pick him up the technician mentioned that possibility.
He had an X-ray and ultrasound and the bladder looks good, urine test came back fine but tonight we noticed blood again :(
Our appointment is Thursday evening with the 2nd vet but I’m hoping to get in sooner. There is a board certified specialist at this place so that’s good.

Oh I'm with you on that! The last, almost month now, has been unbelievably stressful. Most nights have been completely sleepless due to watching EVERY time he went in and out of litter box to see what he does.

I'm happy to hear this 2nd vet is a board certified surgeon - that is so important. They will know more than anyone else what is wrong. I understand waiting till Thursday sounds like a lifetime. If Yuri seems to be peeing even more frequently and/or with even smaller amounts, I would contact first vet and tell them to call your board certified specialist and put Yuri on as an emergency or urgent case. That way you may be able to get in at least a day or two earlier. It's interesting that they are describing it as the hole getting smaller.. In my experience and research, that generally is what a stricture is. The area of urine to come out getting smaller and smaller, therefore less urine being able to come out. But maybe there is another reason for this happening and that is why it's not being referred to as a stricture. I would keep a very close eye (although I'm sure you are already) on Yuri's urinating to see if it gets any worse. If it does then contact both first vet and the specialist you're going to and see if you can get in sooner.

If 2nd vet says same thing regarding revision surgery, I would say in my opinion that it is worth going for it. I absolutely understand the fear of it not working again, especially after all the money that will be spent.. When PJ was brought in for the surgery, I was told there was only a 50/50 chance it would work as with revision surgery there is less skin to work with, as well as chances of stricturing are higher. But If you're going to a board certified surgeon, the chances of recovery are a lot higher. I am so so glad I did it. There is obviously still a chance his recovery will go south, and he will re-stricture.. But I think regardless, knowing I tried, will have made it worth it. Saying that, I do not shame anyone who can't or isn't in the position to do it. I was barely able to do it myself.

You know Yuri better than anyone or any vet - go with your gut and what you think is best for your kitty. :)

Unfortunately we’ve had to come to the emergency vet as Yuri blocked again :( I think he’ll be having the revision surgery tomorrow, trying to be optimistic about it.
