Our cat hasn't eaten in four days, I took him to the vet yesterday and he had extensive bloodwork. Some of his counts were low, but the vet didn't seem to concerned, and one was really low and the vet couldn't figure out why. He prescribed Clindamycin and Metacam, and said if he hasn't eaten by Monday to call back. Last night Spencer started with really bad diarrhea and projectile vomiting. He still isn't eating, and is still vomiting. I stopped all meds, but am wondering if I should still give the Metacam, because he is hiding and just lays there, which could be because of possible pain? My vet isn't there on weekends, and I'm not sure if I should start the pain med again. Hes drinking water well, but won't eat anything, and I've tried everything.

Do you have number to an emergency specialist? I would give them a call. That is too long to go without eating plus throw up and diaheria

Spencer was given Metacam awhile ago because he has bad arthritis from walking bad because he only has three legs. The ER is about an hour and a half away, which I can do but wish my vet would at least take calls about questions. Spencer is acting so listless and hiding, so I'm guessing hes in pain. I'm also guessing there is some kind of blockage, I stopped the antibiotic and Metacam because I was afraid they would make the diarrhea worst. Going to call the ER in the morning, I'm sure they will insist on seeing him, perhaps I will let them do any further treatment instead of my vet. I think tonight I will give him the pain meds, not sure what else to do.

Took Spencer to ER late last night. After 3 hours of waiting the vet there disagreed with my vet about the blood tests. She felt it was leaning towards bone marrow cancer, but wanted to keep him and do extensive tests for a couple of days, totaling almost four thousand dollars. He was so traumatized on the long trip going up there we didn't want to have him stay there for days, so we had her give him fluids. She also gave him an appetite stimulant, new antibiotic and new pain meds. We will call and get him in tomorrow with our regular vet, I was hoping it was an obstruction, but it doesn't look that way. It just puzzles me because he was fine one day, and the next day no eating or walking around like normal.

Wow - that's quite a diagnosis with what seems like very little to go on. Did the ER vet have a copy of the bloodwork? Did they say why they were leaning toward the idea of bone marrow cancer?

Talk over all the details with your vet about the blood work that was done, share with your vet whatever write-up the ER vet did (and they should have done one), and ask what other kinds of tests would be appropriate to start with. X-rays and/or an ultrasound would likely identify any kind of obstruction or masses to at least rule those out as possibilities.

Here is an article on bone marrow diseases, in case you want to read up on it. According to this article, it is very rare in cats.

Keep in touch with er if you have any questions. Disease is something most vets are not as knowledgeable about as a specialist.
As a general rule, ER vets are NOT specialists. It is good to have an ER vet to depend on, but a cat's regular vet is in the best position to follow up on additional testing and ongoing care.

Spencer ended up having emergency surgery on Monday, our vet didn't think he would live if she didn't She was looking for a blockage, but found severe pancreatitis and sepsis. She put him on three different types of antibiotics, and Gabapentin. Brought him home Wednesday, he was eating good, but then stopped. Finally Thursday he pooped, so he started eating again, but no water drinking. Now today , he is extremely congested and is having a hard time breathing through his nose. I don't know if he caught something from the vet, or possibly asperated on one of the four meds he is on. I try to be careful dosing him, but its hard because he foams and spits it out. Once again my vet is closed for the weekend. I refuse to take him back to the out of town ER vet, it was so stressful for him last time, plus I can't afford them. I have been heating up his food, mixing water with it, putting tuna juice in water, etc to try to entice him, but no luck today. My question is, should I still force the three different antibiotics down him when he can't breath, or stop. I stopped the Gabapentin because it said give on a full stomach Hes on Biomox suspension, Enrofloxacin, and Metronidazole pills.
