Soft Poop

Hi! My 8 month kitten had diarrhea when I first brought him home. I took him to the vet and she prescribed a white liquid, cant remember the name of it, and a small packet of probiotic that's to be added to his food every 24 hrs. His poop started to form after a couple of weeks, but now that he's no longer having the medication, it's back to being soft. I'm not sure if it's diarrhea or not. Should I take him back in to the vet?

Has he been de-wormed? What's his diet like?

Yes, he's been de-wormed 3 times since birth. I feed him Blue Wilderness wet food, twice per day, and I'm sorry, but I can't remember the name of his dry food at this moment. It's grain-free though.
