My 4 yr old male indoor rescue cat now has a red spot below his ear. Is this something I should be concerned about? Worth a vet visit? He has no other symptoms, and still plays hard with my other cats.

It looks like he may have caught himself on a corner or something sharp. Gently wash it in warm water to clean the area. Just keep an eye on it to see if it gets worse.
It does look like a minor injury. It could possibly be a little "hot spot." Have you checked him thoroughly for fleas? This time of year, they are bad for coming inside to escape the cold. Hek has a new hot spot. I've treated her for the fleas, and the hot spot.
