Really worried

My female cat whose fixed squat down and the image is what came out of her. She's never done anything like that before. She always goes to the litter box and we got her fixed right when we were allowed to when she was a kitten. My first thought was this looks like blood but she can't have a period. I have no idea what this could be. Has anyone else had this problem? Could this be from her anal gland? She seems normal, nothing has changed about her, but now I'm worried.

Blood = vet visit asap

I'd be calling for an appointment now and hitting the ER if the vet is closed or can't get in today.
