Yes, I do believe if a cat has a fatal illness, they know.
I also believe they know about people, or at least some cats do. Some seem more sensitive to illness in their humans than others, but this is something I have witnessed personality. Something an EMT said to me at the funeral of a loved only made my belief stronger.
