I think we've pretty much exausted all common theories. The vet has confirmed UTI, and his culture is more to validate the severity of the infection and make sure that he prescribes the correct antibiotic.
Fair warning about the following paragraph, I may be repeating information you already know, so please don't take offense. I have seen too many people that either didn't know any better, or that just refused to follow the instructions.

One thing you ABSOLUTELY MUST do is give him the full course of antibiotics; DO NOT Stop the antibiotics unless he has a reaction to them. If he starts acting better and you still have Antibiotics, KEEP giving him the regular dose, unless directed otherwise by your vet. Antibiotic resistant strains of bacteria are caused by failure to finish the full course of a prescription. This not only makes the next infection worse, but quite inevitable. Please, Please, Please Please Please...follow your vets directions as closely as possible regarding the dosage and frequency of the antibiotic medicine. If you have any questions/concerns be sure to contact the vet ASAP.
