sorta random question-do cats ever just have accidents without any deep meaning (stress, not liking the box, illness etc.) behind it? Is it ever just a fluke or something (like a kid wetting the bed)

Yes, an isolated incident can be an accident, according to this thread: Kitten had pee accident, is this normal?

As noted in post #3 in that thread, just keep a watch on him. If he has more accidents, that would warrant a vet checkup. If he seems to be straining to urinate, that requires a trip to the emergency vet.

He seems to be having no problems urinating and is eating/drinking/playing normally. it just seems like ever so often (we are talking like months) he forgets to go in the litter box and makes a mistake and goes in one of the sinks. He is on both dry and canned food for urinary health as prescribed by the vet after he got crystals in his urine and was having problems and started going like everywhere (sofa/bed/sinks etc).
