Probably genetic deformity. I would get him checked at the vet to ensure he's otherwise healthy and your mom should get the momma cat spayed once these kittens are weaned. If it is a genetic deformity (which it likely is) it's most likely to happen again.

Yes I wasnt planning to take more kittens on the cat mom. But I dont think I need my mom to spray her becuase Im 30 years old myself 😂
