Update: The issue has persisted with no change after adding another litter box or changing litter. The vet ordered a feline GI Panel and has prescribed a science diet food.
I'll try another litter box, we use uncovered boxes, one large and one small. The littler is an Unscented clumping litter. We have tried using Natures Miracle on the floor, especially the carpeted area.
