Hermione's Cpk Level Is Elevated

Hey Everyone,

My 14 year old Hermione has had a couple UTIs this past year. In May she was on a couple antibiotics to clear it up, but even after the infection cleared up, she continued to have high urine PH and continued red blood cells in her urine. X-Ray showed no stones.

(I should also mention 4 years ago she had an episode where she was dragging her back legs and lost bladder control. We thought it was a clot, but it was a disc. Steroids cleared it up in a few days, and she's been 100% fine since, but does have some lingering arthritis pain in her back when it's super hot or cold.)

A couple months later, she's doing great. Eating well (waking me up at night asking for food, actually, as she's done on and off since she was a kitten), and running around playing. No sign of urinary straining at all-- and when she was before it was very obvious (staying in the box, crying, tiny pees, etc.). We took her for a check up yesterday bc she lost bladder control suddenly while sitting on the couch, but no loss of control of her back legs like last time.

She hasn't lost weight since May, which is awesome. In May she had lost half a pound and lost some muscle mass. Vet was worried about hyperthyroid or kidney disease, but her blood work for both those things were perfect. Actually, her blood work was perfect BUT she has a very high CPK level-- over 1000. And she still has blood in her urine. PH is perfect, no crystals, and no infection. But with the continued blood in urine, vet is thinking bladder cancer. I tend to think it's more likely inflammation in her back and/or in her bladder causing the blood, the high CPK. I see online that high CPK levels are associated with injuries, inflammation, trauma, etc. but nothing about they being indicative of cancer. Anyone experienced this?

Hi. In all my reading about CPK, I have not either seen it be indicative of bladder cancer. It might be that the vet is leaning that way because of the blood in the urine?

Did the vet take x-rays, or perhaps an ultrasound, to see what the wall of the bladder looks like - and what her disc(s) look like? Inflammation in her discs could cause an elevated CPK. But, with the blood in her urine, it would seem to be more related to her bladder. However, if her disc(s) are bothering her, this might also kick off cystitis due to the related anxiety/stress - cystitis can cause blood in the urine and it does not necessarily always have to coincide with a bladder infection.

The x-rays or ultrasound should also tell if she is by chance retaining urine in her bladder (never fully emptying it) which can cause thickening of the bladder wall and irritation. Ask your vet about this, as this is the only connection I have heard of related to potential cancer - however, I don't think even this isn't typical unless the bladder issues have gone on for years.
