If she's not spayed and has been outside or around an unneutered tom, then she probably is pregnant. Do you have plans to spay her?

However, I don't really see anything about her nipple that seems to be out of the ordinary.

She is not sprayed and she was feral before we got her, idk if she is just fat but she won’t let us touch her stomach and all she does it eat and sleep. She will play around but only for a little while.

Her nipples don't particularly look any different to a non pregnant cat however the fact she ain't spayed means there is every chance she could be pregnant and maybe only a couple of weeks.
I'd get her off to the vets and booked in for spaying.
Good on you for taking her in though

I'd just book her in to get spayed, many vets wont spay if they are pregnant or they will contact you to ask if you wish to proceed.
