I think she turns to chewing on strange things when she's nauseated. I think carpet fibers are a cat grass substitute for her. When I had cat grass available to her, she didn't seem interested in it until she was nauseated. I don't know if I can keep grass always available to her just for those odd occasions she's feeling nauseated. Though if it keeps her away from the carpet fibers, I might have to.
Edwina has sometimes chewed on carpet fibers, too. She also chews on electrical cables and cords. And loves to lick plastic bags.

It really seems like she has something stuck and nothing wants to stay down today.
I hope it goes well! Fingers crossed again!

She only eats weird stuff when she’s nauseated. 29 days out of the month she may be perfectly well-behaved in one she eats and doesn’t eat. And then she gets a brutal hairball and the nausea that comes with it and I feel like I have to watch her like a hawk until it’s all passed.

Ugh!

Because it was only supposed to be a recheck appointment, they didn’t allocate an exam room for me to meet with the specialist. I had to give as much info as I could to the nurse and then let her whisk Betty away to the back. The specialist will call me when he’s looked her over and I guess we’ll go from there for what else might be in store for her today. They told me I should have called earlier this morning since it’s no longer a recheck appointment.

Okay. Just got off the call with the specialist. Apparently Betty now had some diarrhea in the exam room. The specialist is going to give her some metronidazole and bam! There goes the last month of AnimalBiome progress. 🤦🏼‍♂️😿 Maybe the metro will help with the unfriendly strains in her gut. We had quite the argument where he’s pushing steroids once more and I’m saying,”please. Let’s look for a foreign body ingested. Let’s do the necessary for that and we can discuss steroids another time.” I’m starting to lean in the steroids direction since we keep having episodes, we have a diagnosis through imaging and biopsy, and I don’t know how long it will take for AnimalBiome alone to do its thing. Especially if she’s getting antibiotics like today. 🤦🏼‍♂️ It’s not fair to her to keep withholding treatment and pretending she’s doing better than she is on alternatives. As the specialist says, it’s likely the supporting medication is blunting the worst of it. But if she was really doing as well as I believe, she wouldn’t need these meds. And I wouldn’t be reporting, “well she is still having hairballs, but not as frequent or not as bad.”

I don’t know when we can start the steroids. I don’t know if we can get her off the mirtazapine first. I’d hate to have her on both at the same time. And I hate that I can’t just have a face to face conversation with the specialist right now. We just talk over each other on the phone and I don’t feel like I’m totally being heard. I hate what this dumb pandemic has done to vet care. 🤬

I’m coming to the point where I think it’s probably about time to start her on steroids. I don’t want to do that today. And I didnt get an answer on how to balance the steroids and mirtazapine or how to wean her off the latter to start the former. If he wants me to start her on steroids, we need to have a better plan than this.

Can you get whatever it takes in their world to actually talk to someone to voice your wants and needs for Betty?
I feel the same after a dental today. They had the appointment time wrong, kept me waiting for what seemed like forever, and then I get a hygienist who seems not to have even graduated, didn't have my chart and acted surprised when I told her what I needed and asked her to get my chart so she could see for herself.
After taking forever with the cleaning, I get up to the desk and it's suddenly $25 more.
So I empathize.

Sorry this is happening. Keep in mind that the steroids might help to stimulate appetite so you won’t need the mirtz. I agree that it would be a lot for Betty to be on both.

Okay. They did shuttle me into an exam room to speak with the specialist face to face. No evidence of foreign body ingestion on the X-rays or ultrasounds. She did have a messy diarrhea blowout in the office. Thankfully she was not in her carrier. I asked that she be sent home with metro rather than giving it here today. I’ll double her Gut Maintenance dose which is what she should have been taking in the first place but I didn’t want to push so many capsules on her. We’ll also open a new case of I/D just in case we got a bad case there. We got a bad case of A/D the other week. The specialist wants me to start her on steroids sooner than later. He also said that we shouldn’t wait for the mirtazapine taper. But do them at the same time. He suggested a taper schedule of extending her to every third day. Since she’s probably still “off” tonight, we’ll skip tonight’s mirtz dose. Maybe we start the steroids on Friday. I want to see if she can get through last night/today before I throw another wrench in the works. I also wanted to make sure we have a plan with both the mirtz and the pred before adding more crazy.

She’s busy grooming herself after her blowout. I think I’ll take a walk and give both of us a chance to decompress and give her injected Cerenia a bit longer before I start her with a trial portion.

Yeah. I didn’t want to start her on steroids until AnimalBiome had a decent shot to work. But between the flox and possibly metro, we keep hitting an unhelpful reset. It’s not fair to withhold a treatment that likely will work on the hopes of a treatment that may not be enough or may take too long.

And yeah. Fluids, ultrasound, X-rays, lab work, and meds came out to a $1700 visit. 🤦🏼‍♂️😿💸

Oh---this is not good.. Betty is really sick..
What I am about to say is probably not what you are wanting to hear, but here goes.. Bear in mind, I have medical experience.. so that may color my opinions.

While I applaud you for trying non-traditional approaches to IBD; I really feel as the Vet does~~ Betty seems to need traditional medicine at this point; and steroids. I know you are trying a more natural approach, but it may not work with all cats. Since Betty is new to you; you do not know what occurred in her previous home. Also, she keeps having setbacks. If the non-traditional stuff was helping her, I am sure you would have seen a large improvement by this point.; yet, you have not.

To me, it seems the AnimalBiome's reporting system is very late in reaching you. Yes, it is a lot of information; however, by the time you get the report, it seems you are already purchasing another order from them. That kind of had me wondering, from the beginning.

I think for Bettys sake, you should probably stop the non traditional stuff; and start the steroids, and whatever the Vet suggests.. You can always go back to the Animal Biome stuff at any time.. and the rest of the non-traditional stuff. See how Betty reacts to a traditional medical approach. You may be surprised.

The steroids should improve Betty's appetite. With the Mirtz, from my own experience; you could use it for a bit, then taper it down, slowly.
You can keep the probiotics, or what they are calling it. It could probably work..

I am really sorry you had such a terrible experience at the Vets. Yes, Covid makes it all difficult. I am surprised they are back to the full Covid protocols. WE have not hit that point, yet. I was just in Angell, yesterday.. masking is optional.. they do provide them. I masked and walked right through.. no restrictions.

I think Covid has caused vets to endure incredible stresses... in addition to all the issues with Covid, (staffing, phone call appointments after examining an animal) to increased case loads, supply issues, food issues for prescriptions, burn out, and many more sick animals and owners; that they have to deal with. I know my IMVet and his staff are so busy, that they prefer emails rather than phone calls.

Ugh Veterinary care is so dam expensive. Not regulated, and so expensive. I hate the soaring costs of it.

Do you think Betty could have a chicken allergy? just a thought. I have had 2 cats with chicken allergies.. I am sure, after all of Krista's issues, you are aware of all of this.. but.. just a thought.

Sorry this is happening. Keep in mind that the steroids might help to stimulate appetite so you won’t need the mirtz. I agree that it would be a lot for Betty to be on both.
Mirtz is hard enough on Betty that I don’t want her on both. But we may not have the luxury of weaning her off mirtz before starting pred. One of the goals of today’s visit—before last night and today happened—was to learn how to taper her off mirtz. Mirtazapine is an antidepressant with a munchies side effect. It can cause her trouble and grief to stop mirtazapine without a taper period. Since last night was awful enough for her, instead of giving her a mirtz crazy night, I’ll skip tonight’s dose and start her on an every third day taper as suggested by the specialist. Perhaps we start her on pred on Friday night so she doesn’t get the double whammy her first pred dose. It would also give her a second pred dose before the double whammy night. Probably not enough to acclimate. But two days is better than one.

No I know Betty isn’t doing as well as I had hoped. And I know we should try the steroids already. I just need a taper plan for mirtz and I got that today.

I’ll give her an extra s boulardii tonight for the diarrhea she had earlier. I don’t want to start her on metro unless that diarrhea persists.

I still believe in AnimalBiome. But I think it’s going to take a lot longer than I was hoping. She does do better taking it than not. And if we don’t need the metro, there’s no reason to stop giving it. It can work in conjunction with the steroids.

Even though this doesn’t present like Krista’s did, it also isn’t managed like Krista’s was before we had to go on to steroids when it was no longer managed through diet and alternatives. Betty’s bowel disease has never been truly managed and that’s concerning. Runaway IBD can progress to lymphoma.

And yes, I considered that chicken may not be a 100% safe protein. But if she had a chicken allergy, I would expect it to affect her more dramatically and consistently. Krista couldn’t have chicken and she always let me know it if I slipped up. And if she can’t have her I/D, what can she eat? She’s never shown enough interest in any other food. Except Fancy Feast which I suspect was causing her more grief than relieving it. She didn’t like the turkey FF and all the others have chicken or fish ingredients. Hopefully the pred will open up her palate. We’ll see. I feel like we have been making progress on her diet. She’s no longer eating an A/D mix. And she’s eating enough I/D to maintain her weight. Neither of those would have been possible without the AnimalBiome therapy. But AnimalBiome is taking too long and it’s probably not powerful enough alone. So we’ll enlist an ally. Maybe the combination will put Betty into the 25-30% of cats that are able to get off pred.

Rabbit. And maybe one or more other animal proteins you haven't tried. Duck? Venison? Quail? Lamb? Beef? and there must be others as well.
So sorry that all of your valiant efforts have not thus far resulted in a consistently healthy, happy Betty. Praying and hoping that you and her carers will find a combo that works. It is the worst time in modern history for this to be happening. I know.

I believe in AnimalBiome too. The curses are as mentioned you not knowing her previous history and then -arg- these unasked for but probably needed antibiotics. Unless the vet says otherwise, I don’t see why you can’t give her that stuff alongside. I do worry about your lack of sleep and just YOU in all of this…maybe you need to keep it as simple as you can for awhile, so you get some rest (which also helps Betty). My vet says with pred the simpler you can keep what goes in a cat, at least to begin with, the easier it is to eliminate what might cause the IBS in the first place. (Do you think there is any chance at all any of the biome stuff could be an irritant in her system?). You have been such a trooper in all of this, was really hoping this might be a science breakthrough (and still am). When I took Zorro on a supervised walk to the garden he was so excited, immediately munched grass and rolled happily in the dirt. I do believe there is something they get “out there” that they know is good for them…like little kitty herbalists. Anyway rough day for you both, but tomorrow is a fresh start. One constant in all of this is your fierce devotion to each other, which is a joy to read about.

And head shaking is a rare side effect of ondansetron. But no, I think there’s something going on with her ears because if I scritch them or she bunts me, she’ll frequently follow that with a head shake or an ear scratch. I talked to the IM and he said the ears are a lower priority right now.

I suspect she was a kibble kitty before I got her based off her eating habits, her kibble preference, and her initial gut biome report. In that sense, I don’t think I get enough credit and especially not with the vet or the specialist for getting her off kibble and onto scheduled feedings rather than two plate drops and hoping for the best like when I first had her. We have made tremendous progress. Sadly, there’s more needed that I don’t believe AnimalBiome will do alone.

I’ve been trying to simplify. I got her down to eating I/D alone rather than mixed with A/D. I wondered if she needed Optagest (digestive enzymes and prebiotic) when I switched her to I/D alone. That was something I thought I would try to simplify. I did not realize how carb heavy even the I/D is. Perhaps the two week break from Optagest is part of her back slide. The trouble with Optagest is that it turns food soggy the longer it sits in those digestive juices. It made the AD mix unappealing. Maybe the wheat gluten and other thickeners in the I/D will help it hold up better if it sits in Optagest for awhile.

I discontinued the famotidine just as soon as her endo ruled out an ulcer. So that’s another thing we simplified.

I don’t dare take away her ondansetron yet.

She looks a little dicey right now. She made such a loud fuss that she ended up getting three trial portions in three hours. She’s finally stopped pacing, yelling, and hunting (she goes up to the counter and checks if I left any food out in her prep area.)

I think we’re both watching and waiting to see if this stays down. Oh good. She has finally laid her head down. I mean I’ve seen that before and she’ll pop up nauseated. But hopefully, for now, I’m hoping the injectable Cerenia and the ingested ondansetron will keep everything down tonight. I gave her two s boulardii capsules tonight to help with her diarrhea from earlier. The specialist and the tech both made it sound like I should expect more. But so far, that hasn’t proven out. I wonder if it was simply a matter of her two hairball treats from earlier on a mostly empty stomach.

So the plan for the rest of the night is half portions. If no further incidents, she can resume normal portions in the morning.

Her head just popped up and she must be contemplating whether and how much she might be nauseated. 🤦🏼‍♂️😿

The plan for the week, assuming no further acute incidents, is to resume her next mirtz dose tomorrow night (having skipped a dose tonight) and we’ll most likely start pred on Friday night. She’ll get two pred doses before she gets the double whammy of pred and mirtz. We’ll try mirtz every third day for a week. And if her appetite holds, we’ll try to move her to every fourth day for a couple weeks before trying to discontinue it altogether. It will be about this time that we do a pred recheck to determine if she can go to an every other day schedule. At least that’s what it seemed like he said. He will be calling either tomorrow or Friday to check up on her. I can run the plan past him again at that time.

Gosh, I wish she could get comfortable tonight. I don’t want her spitting anymore. And I don’t want to watch her like a hawk all night. I just want her to go back to eating and sleeping. And sometimes cuddling in between.

She made it to second dinner. I plated 20 of her usual 36 grams because she tells me those 14 gram portions she got earlier were, “🐂💩!” Then she proceeded to eat about 17 or 18 grams of it. I’m okay with that. That means first that she’s finally filling up. And second that she’ll have a few grams before next meal.

I added the Optagest back in knowing that this portion would disappear in short order. I put Optagest in the “might help, probably won’t hurt” category. Given her last night and day, she could use a gentle ally. Other than making AD soggy, it’s our easiest lift of all the supplements. She doesn’t seem to mind it and it has helped her before. Back when we were making the AD mix, I tried to take her off the Optagest and her hairballs came roaring back in full force. After I added the Optagest back in, the hairballs didn’t disappear. But the frequency and severity seemed to improve. Maybe that’s what happened here when I switched her to all ID and started skipping the Optagest. Oh well. I’m not going to put off the pred testing out that theory.

So I’ve been on diarrhea watch 🫣 since they gave her back to me. If they hadn’t told me, I’d probably not know. Not only have there not been any rivers, lakes, or puddles, she hasn’t even pooped since she got home. If the diarrhea was from an infection, I would have expected to see not just loose stools but frequent stools as well. I know when Covid got into my gut, I was pooping every hour whether I had something or not. 🤦🏼‍♂️ I’m glad I held off the metronidazole. But I still have it on hand if I’m proven wrong later tonight or tomorrow

She seems a little restless. And there was a tense moment earlier when she kept going to the fountain. I stopped her short each time. “Don’t load up!” Because she doesn’t need it. She had fluids. And she has a tendency to drink too much water when she’s not feeling well. It’s almost like she uses water to load up her stomach and bring whatever back up. Liquid cat grass.

She skipped after dinner cuddles with me. But it looks like her head is finally down. For how long….? 🤷🏼‍♂️😿

Despite her restlessness last night—she put her head down, then lifted it again, then down, then lifted again like she was having trouble relaxing—she did finally get sleep last night. And while her portions were reduced, she turned in clean plates all night. Breakfast is nearly in the books too. I plated 31 out of her normal 34 grams and she ate probably 26 of that in one go. The Optagest also makes I/D soggy but it seems like she overcame that quickly enough. Only a handful of bits remain.

I only gave her one hairball treat this morning. It is a new bag. And the only other time she ate from this bag, she had a vet office blowout. So let’s see what one does to her before we get her back to two.

She had a pretty angry and irritated scab below one of her nipples for about a week. It was one of the “put it on the list” items for yesterday. Whatever he put on it yesterday, it looks so much better today. It’s looks like the scab fell off, and the rest of the site looks to be healing nicely.

That’s good news. We can use some good news around here.

Still on poop watch. I know it may not arrive until later tonight based on how little she kept down yesterday and the night before. But both Dr and tech made it sound like a tsunami was coming down her pipes. I’m still glad that I held off on the metro. Don’t make me regret that, Betty! I have a good feeling it will be a solid poop with a squishy piece. She always has at least one squishy piece that I think is part hair and part hairball treat.

Absolutely we can! Wonder what he used. Wonder why she had that little injury.

Let's hope for no surprises except perhaps good ones.

Dr said 🤷‍♂️ .

He said maybe her gut was hurting and she overgroomed herself or maybe who knows? Maybe it was a bug bite that she overgroomed. Since she's not talking and it's only that one place, there's nothing we can really say for sure. But whatever he put on it or did for her, it looks so much better today. I don't have to shame her on the Internet with nip pics. 🤦‍♂️😾
