Hi all – I had started a previous thread about my vomiting cat. He is 16 years old and had acute pancreatitis back in November of 2023. For the past 2 and a half months he has been vomiting every morning between 5am and 6am, a small amount of partially digested food. He has had almost every test possible and two ultra sounds. Luckily all is well so they concluded he has IBD. My vet (and the internet) suggested Famotidine. The vet wanted to send a prescription for $72 (which is insane) so I got it over the counter and gave a half pill, 5mg on Monday morning. He vomited a small amount of foam an hour later and would not eat for 6 hours. He would go to his bowl and stare at it and walk away. When he did start to eat later he finished his wet food and nibbled all of his dry food by bed time. I switched to hydrolyzed dry food but it’s only been a few days and I am still transitioning from his old food. My question is regarding the Famotidine - should I give him 2.5mg twice per day? Or the 5mg once per day? Or 2.5mg once per day? Can I give it in his wet food? He’s tough to pill so ideally, once per day and mixed with wet food would be best, but I need to follow whatever is going to help him. Anyone have experience with antacids? Does it take them a few days to get used to it? Is loss of appetite common? I just got him eating properly again. A few weeks ago he was on Cerenia which stopped the vomiting but he was super lethargic and would not eat at all. Once I took him off it, he went back to normal. I’d hate for my choices to be him vomiting every day (which I assume takes a toll) or him barely eating when he has meds to help.

He prefers dry food oddly, so feeding only wet wouldn’t work out and he does seems to have smaller amounts of dry food all day long.

Any help would be greatly appreciated.

Zofran (Ondansetron) is actually more effective because it works longer (12 hrs) and each pill is equal to 4 doses. Famotidine is a shorter acting (6-8 hrs) antacid. You want to give 1/4 (2.5mg) of a 10mg tablet twice a day.

If he'll only eat dry food, check out Young Again Zero for a great option for IBD kitties.

Thank you - does the Zofran decrease appetite? He was definitely not hungry after taking the Pepcid and it took 6 hours for him to eat again. I just bought the Purina Pro Hydrolyzed. Do you know if hydrolyzed or a single protein like rabbit works better?

Sometimes cats still can't tolerate the hydrolyzed protein foods, but it does take a few weeks after being on the new food to tell. I think if he's still getting sick after 6 weeks, then you may want to try a novel protein food.

As far as the famotidine, they can still have an upset tummy depending on when it was given and when they ate. Although I never had a cat that was inappetent after taking it.

Zofran works differently, and shouldn't impact the appetite. A lot of cat parents on here prefer Zofran to Cerenia or pepcid, as it's less side effects.

You'll find a lot (a lot) of pet parents on here have furry kids with IBD issues, and have been through trial and error with a lot of these things. You'll find a lot of resource and experience in these posts!

Good luck with everything, hopefully you find a combo that works best for your buddy.

Thank you so much. It does seem to require a lot of trial and error so i will give a lower dose of Pepcid over a few days and see if he improves. If not, I'll ask his vet about Zofran.

I appreciate the help!!

Zofran doesn't decrease appetite, in my experience, but nausea does. They appear to eat, but then don't eat because their tummies are upset. I would also request an ultrasound to make sure you're dealing with IBD and not something like pancreatitis or gastroenteritis.

Hi - he has had 2 ultrasounds and everything looked healthy. The Cerenia cured the vomiting but he did not look good - poor guy - head hanging, eyes wide and would not eat. As soon as I stopped it he starting eating and was acting normal; again, but the every morning pukes came back. I'll give the food a chance to work and will try a lower dose of Pepcid. If all else fails, I'll ask the vet about Zofran.

Hi - sorry for the delay - I was asleep and then work had been crazy. I tossed the box, but here's the bottle. Generic Famotidine, 10mg, from amazon. I cut it in half originally and gave him 5mg. Last night I gave a quarter pill with his wet food. No vomit this morning, but we'll see when I get home. Thanks!
