3mo kitten with pneumonia - Praying for Survival!

Of the kittens I've fostered so far this past year, 3 at different times needed supportive care, fluids, IV antibiotics, and syringe feeding within 3 days of taking home to foster from the shelter. They were all labeled "fever of unknown origin" and treated by 2 different vet hospitals' internists initially. Two were 104 for 2 days, one was 105 for a day before starting to consistently decrease to a normal body temp.

2 of them were spry by day 2 on fluids, needing Ecollars to keep them from chewing their lines and catheters per their ICU nurses! They (the vets and nurses) say fluids make a lot of pets with fevers feel spry within a few hours. One of the 104 kittens, younger and smaller, was very lethargic for 3 days before improving. The regular vet of that particular rescue organization had some poor initial antibiotic recommendations, with only split Clavamox tablets (all out of foil, split and in a bottle...they had "spoiled" and were yellow from exposure) and a single dose of SC fluids as her initial treatment. The other two started with the specialty place on azithromycin (only avail as oral per the vet) and unasyn (broader ampicillin, a penicillin) IV, as well as their IV fluids immediately and were bouncy within hours in their supposedly "hot, lethargic" state, hahaha! They still had fevers but gradually decreased over 2-5 days. Only had those 2 on IV fluids for 2 days and nights, then home on azithromycin. One of them had a specialty doc that considered no AB to see if the fever was only viral in origin and could be treated with supportive fluid care only, but then went ahead with AB; I've had internists go back and forth with starting AB immediately vs waiting to see if resolves on fluid and saving initial swabs of nasal passage to culture for effective treatment before starting a general AB that may affect culture/sensitivity results.

The smaller younger kitten was hospitalized a total of 5 days before home nebulization, oral azithromycin and Veraflox, fairly new and very potent URI specific AB, syringe feeding, proviable  probiotic and dewormers were continued at home. They cancelled the culture, as the result wasn't available yet by the time they were releasing her back to me, and the treatment was working; the rescue would have continued authorizing it if she had not improved so well by day 5. Their bacterial infections were all secondary to a primary virus per the different internists, and this is common in shelter or rescue cats coming from high population/traffic confined areas they both said.

Oh, and b/c of that , they're all on l-lysine amino acid twice daily too, to limit the virus' replication; both docs assumed herpesvirus as this is the most likely culprit that made the bacterial infections leading to fevers possible. Only one developed thick green discharge, as the antibiotics were started so soon when they had fevers. nebulization kept them all draining and clear, helping keep their appetite up with a good sense of smell, for about a week after they were released from hospital.

Good luck to your Zoey and just stay the course, even with ups and downs in fever and appetite unless she consistently doesn't want to eat or is lethargic for at least 8 hours. The subcutaneous fluids should be repeated in my experience so far, if she isn't going to be in hospital on IV fluids, as this is what helps decrease their temperature and hydrate them while fighting a fever. I took the straggler kitten in daily for outpatient SC fluids for 3 days while her fever was slower to consistently decrease, and she was always brighter afterwards, eating and zinging about at home, popping the older cat's paws if he stuck them under her door to "play."

When was the last time her temp was taken and are you doing it at home.  104 is a high enough fever to make a cat act ill and lethargic.  If you're not temp'ing at home, you may want to consider doing that.  It isn't difficult, you buy a digital thermometer at a drug store, the 8 second ones are the fastest you can get and some lubricant (KY).  Here is an instructional video.

Today's Zoey fever came back down. It definitely seems like the SQ fluids help fairly quickly. I've been asking the vet to give them to her 2x a day as I don't think she's drinking enough at home to keep her fever at bay. Zoey's been on azithromycin and Clavamox for the past 4 days and will be doing another X-ray on Wednesday to see if it's working. My vet did talk to me about starting her on marbofloxcin or Baytril if her X-ray shows no improvement in her lungs. I'm still nervous about the long term side effects as she's still growing and I don't want to potentially damage her joints and/or cartilage. Does anyone have experience with the fluoroquinolones in kittens? Lots of success stories with cats but how about a little tiny kitten?

And just when I thought my poor Zoey couldn't be faced with anything else, the vet told me today that she now has an intestinal parasite called coccidia. [emoji]128563[/emoji] I don't know how this is possible considering she's been given 5 different dewormers, was negative on her fecal test last week and didn't show any symptoms. Stools have been normal and she's has no vomiting. I'm going to ask our vet tomorrow as the vet who "diagnosed" Zoey today was not our usual vet so I'm not sure if she truly understands Zoey's history. How can she have been given so many dewormers meds and have 2 tests come back negative then all of the sudden shows up with this infection? I looked online and it says the incubation period can be up to 13 days which would be well past the time we brought her home from the shelter. We adopted her on July 18th. I'm now wondering if she picked it up while traveling back and forth between the vet and the emergency clinic. I just don't understand how she contacted this parasite all the sudden. They started her on Albon for 11 days. Is it possible her recent fever spikes have been caused by this parasite? Zoey's such a loving, sweet little munchkin...I just want her to start feeling better. [emoji]128591[/emoji][emoji]127995[/emoji] She's had a tough start on life that it's time she start feeling better and being allowed to be a kitten.

On the one hand Coccidia is not a worm and isn't affected by de-wormers, on the other hand it's usually diagnosed in the presence of bad diarrhea, so unless you have a bad case of diarrhea, i'm not sure I'd be very worried about it, and if i was going to be worried about it i would insist on a PCR (polymerase chain reaction) test, which is totally realiable, not very expsensive and only takes a couple days at an outside lab like Idexx.  It's the gold standard for testing of this type and i've done it many times with cats, totally reliable.  Nothing else counts.See http://www.zoologix.com/dogcat/Datasheets/Coccidia.htm

She looks bright enough but with young cats it is difficult to tell. Hang in there Zoey. I am praying to the cat Goddess.

WOW!  Poor Zoe and poor you!  We went through coccidia with Hannah.  I'm praying for her continued improvement.

Wednesday Zoey Update...
It was a hard day for everyone today. After the last 2 days of being active and finally acting like a kitten, I noticed last night that Zoey started coughing again. Of course my heart stopped when I heard the first cough! Unfortunately throughout the night Zoey coughed a few more times and this morning woke up sounding like she had a rattle in her breathing. [emoji]128560[/emoji] She was scheduled to have another X-ray and CBC today to which I was hoping would show improvement in her pneumonia. I wasn't praying for miracles but based on her behavior I thought for sure we'd get good news. Sadly this wasn't the case. [emoji]128554[/emoji]

The pneumonia in Zoey's 2 right cranial lobes appears to have cleared, however those lobes are still clasped and have not reinflated. The bad news is the pneumonia is now showing up in her left caudal lobe with a slight presentation in her right caudal lobe. Her white blood cells have increased from 19k a week ago to 33k today. A sure indication that her infection is still very much present. The radiologist mentioned possible COPD and or future asthma as long term side effects. He recommended removal of her clasped lung lobes once the infection is cleared. OMG! How can I put a 3 1/2 month old kitten through that kind of surgery?? [emoji]128560[/emoji][emoji]128560[/emoji] We decided to switch her meds today to Orbax in addition to the Clavamox. According to Zoey's culture report Orbax is a susceptible drug for her pneumonia. We're praying this is going to work and clear her infection. Our vet said if this doesn't kill the bug then we need to see a specialist.

I'm so broken hearted over today's news! Zoey has been through so much in her short little life. She's a true angel and is continuing to fight with everything she has. I pray that she pulls through yet another set back. She's coughed a few times tonight which is painful to watch. What makes this so hard to understand is that Zoey's been eating, drinking and playing like a normal kitten. She's gained weight...up to 3.7lbs, and has normal litter activity. She follows me around the house, purrs when I pet her and cuddles on the couch when watching TV. If you didn't know the back story you would never guess she was so sick. My vet said Zoey may have chronic pneumonia which was passed on from birth. I've never heard of that and don't know the long term effects but I would imagine that's not good. I'm praying the Orbax will be the medicine we need to knock this infection. I'm also hoping that Zoey's lung lobes will regenerate healthy tissue once she gets over the infection. My vet told me it was a long shot but hey, a week ago no one thought Zoey would be here with us and look how far she's come. Please pray for Zoey. The love, thoughts and prayers have helped get her through this far. We need them now more than ever with this latest relapse. Thank you StephenQ for the suggestion. What's crazy is Zoey never had diarrhea nor vomiting. I was really surprised when they told me they found it in a stool sample. From everything I read, Zoey had no classic symptoms. It's very strange.

That's heartbreaking to read. I have a sick cat I'm caring for and I thought we had it bad already, but this is way more terrible.

I can't say much other than I know how it feels when a pet suffers and you seem helpless to do anything. At least Zoe has somebody to take care of her and I wish her all the best

I'm crying and praying at the same time. Poor little girl what a rotten start in life no one deserves all she has been through. Hang in little girl and Mum as well you must just about be punch-drunk I'll bet you've not had one good nights sleep since you brought Zoe home. You would be counting days and nights one hour at a time. Please whoever looks down and cares about baby kittens help Zoe. anneno2

Hey it isn't crazy, plenty of cats don't have symptoms even though they test positive and as this site suggests you may not need treat if there are no symptoms.  See this link and talk to your vet.

Poor Zoey. The good thing is she has a home and she has you. I really do not think she would have made this much of a recovery if she was still in the shelter. She has gained weight and seems to enjoy life. That in and of itself is a good sign. Orbax is a pretty broad-spectrum antibiotic, they say most cats like it, one of mine when he was put on it did not. Be sure to shake it well as it is gritty. Please keep us posted on Zoey. She is in my thoughts and prayers.
