13 yo cat eating less and less food. Losing weight. Vets can't figure out why.

My 13 yo neutered male cat has always been a walking garbage disposal eating whatever we put down for him. He subsistence on a reduced calorie dry mixed with water, Fortiflora, and a cosequin as he got older. Gradually, he started eating less and less. Now, he acts like he is hungry, but only takes a few bites of even the yummiest, stinkiest wet food, or even people grade tuna and chicken before shaking his head to spit out the last piece he grabbed, then walks away. He knows I will then try to feed him a buffet of other options to tempt him into eating a bit more. He sniffs and usually tries at least a lick of them, but eventually gives up and leaves. Its like he's waiting for me to give him something that agrees with him, but when he realizes I can't, he leaves. I've tried every brand and flavor wet and dry, meat baby food, high calorie prescription diet, human chicken, tuna, and salmon, CN canned from the vet, and lots of gravy toppers and wet and dry treats, but nothing holds his interest for long. BUT, then he'll come back to the kitchen an hour or so later acting hungry, and we go through the whole process again. He probably ends up consuming a few tablespoons of food each day like this,, but I have yet to see him drink water. I do mix water and fortiflora with the wet food i offer him but am not sure that it's enough to keep him hydrated. His skin does not stay tented, and the vets only gave him fluids once to keep the Cerenia from stinging, so I don't think he is dehydrated, but is this really enough liquid for a now-11- pound cat?

He has been seen by two different vets, had bloodwork done twice, and a thorough physical exam. Also had xrays. All were completely normal. Also did a thyroid test which was also normal.

He's lost 2 pounds in two months. He had a full dental two months ago, no extractions (I brush his teeth). Vet said all his teeth and mouth look "pristine".

He has had no vomiting, no retching or hairballs, no diarrhea, no tarry black stools, no constipation, completely normal poops and pees. Still using the litterbox perfectly.

Vets have given him Cerenia for nausea, Convenia injectable antibiotic in case he has infection somewhere (has no fever, no elevated white count), a non-steroidal anti-inflammatory in case it's the start of IBD, and Gabopentin painkiller which i give him twice a day,....even though he does not seem painful we know cats hide illness and pain well well. He also got Metronidazole appetite stimulant, and now I give him a transdermal version of this on his ear pinna once a day. None of this has improved his picky eating.

The problem to me seems that he is not really anorextic-- he acts hungry, like he wants to eat, but after a few bites the food is no longer palatable to him and he stops eating. Could it be upsetting his esophagus or stomach?

The other problem is lack of drinking water which he use to do especially out of my water cup whenever I would refill it. Can he possibly be getting enough fluids through the watery wet food does eat? He is very difficult to pill and inject so I'm not sure he would be a candidate for force feeding food or water or subq fluids at home.

He's always been a super healthy cat and a foster ambassador (loves other cats). He definitely got depressed when we lost his 18 year old "sister" after a long battle with IBD, but he never went off his food. I've had two fosters come through the house since then and that seemed to get him out of his depression-- perked up, started playing again, etc. I have adopted my most recent foster, a 4 year old friendly girl whom he loves (grooms her when she lets him).

What else.. I had to moved to Colorado for a month this fall to help with a family crisis but I had lots of people visit him twice a day to feed and play with him. He never went off his food then either, and he loves visitors. Now the house is calm, nothing to freak him out, plus he's not a freaker- outer anyway--loves people, loves cats, never met a stranger. Just a happy go lucky, very mellow lovey boy.

Any thoughts on this recent aversion to food and lack of drinking with no other abnormal symptoms or labs? We're going for ultrasound next week. Should I ask for gastric endoscopy too? Could it be ulcers? Thoughts?

Thanks for any input. This boy is so special to me, and he has helped so many frightened fosters become confident housecats thanks to his kind nature with other felines. It's just too soon to lose him.

Do you mean mirtazapine for appetite? The other is an antibiotic.. I had a rescue kitty who did something similar - she was just wasting away for no reason. The vet threw up her hands and gave her a huge dose of multiple kinds of antibiotics, and a steroid, and said, 'cross your fingers' - and it worked. No idea what it was or why, but she just started getting better, and is now spoiled rotten, since I was able to rehome her to a very affluent family who adores her and treats her like one of the family.
Have you tried elevated dishes? Will he lick the food off your fingers? Just a few thoughts. :) Good luck!

If he loses all desire for food the biggest concern is hepatic lipidosis or fatty liver disease. Another possibility would be pancreatitis. Do you know if they did a fPL test with his bloodwork?

I know you saw two different vets but were they from the same vet clinic and were either one of them a feline specialist? If not, you may want to consult a Veterinary Internal Medical Specialist. Best of luck, please keep us posted on his progress. :crossfingers:

Do you offer tap water or a bottled/filtered. My cats don't like the tap water in my city at all and won't drink it. I have to boil/filter it first or give them bottled water or well water from a family member with a well to get them to drink more than a few kitty sips.

Another idea to get more fluids in him is to offer some ice cubes he may lick on them a bit and get some extra water. Another trick I did was add some canned tuna/chicken to water and offer it. My ill little girl went for that every time. My boy not so much.

Though he may be getting just enough hydration from the watery canned food as well. If he's not that active anymore, that may help. I noticed my cats drink less now that they're older and mostly just snooze the day away.

It may take a while and trying all kinds of different things to get him to eat and drink well again.

Those tests will absolutely be the next steps if ultrasound and endoscopy don't reveal any pathologies. He does have chronic sinusitis which is controlled by Convenia when it flares up. We've already done xrays, evaluated by radiologist, results "unremarkable".

Oh and I do serve filtered water from my fridge dispenser. Out tap water smells like chlorine! If I tried to feed him tap water, he'll search out a cup of water i am drinking from and consume that instead!

To whomever suggested kitten food, THANK YOU! He didn't eat all of it but he consumed more of it than anything else I have tried so far, including human chicken, tuna, and salmon! Fingers crossed.

If your cat has no other signs pointing to IBD other than inappetence, if it were me - strictly as a lay person - I would be starting with an investigation of his nasal passages/etc. before I would worry about an (abdominal?) ultrasound, or a gastric endoscopy. If he has abnormal tissue growth somewhere in that area - which can happen with chronic sinusitis, I am not sure that would show up on an x-ray. But I am sure it could be rather bothersome to him.

It might also explain why things like Cerenia, Mirataz, antibiotics, and anti-inflammatories might not be helpful. Have you had a discussion with the vet about what chronic sinusitis could cause over the long haul?

Glad to hear he at least liked the kitten food!!

My cat is very social and recently I noticed he will wait to eat until I’m around. So I have made a routine now to sit with him while he eats some of his meals. he eats a ton more! Crossing my fingers for you that you find something

The two vets were both at the same practice yes, both were the practices "cat vets" but I dont

The reason I asked if the 2 vets were at the same clinic is because you're not assured you will get an unbiased independent evaluation/diagnosis if they're in the same practice. That's why I recommended getting a second opinion from a feline only or veterinary internal medical specialist. Best of luck, glad to hear he ate some of the kitten food. :thumbsup:

I agree that is a very good point. I just made an appointment with our local specialty-laden emergency 24/7 vet hospital. Extremely talented vets, huge state-of-the- art hospital, but Ungodly expensive just to walk in the door. But I don't care. Too soon to lose this special boy.

UPDATE: Elevated dish seems to be helping! I've only recently learned about whisker fatigue and bowls and plates that can help. Not sure how true it all is, but it does make sense. In those same threads, I've seen cat owners swear by elevated and/ or angled bowls which seemed weird. I mean, cats eat their prey off the ground right?

Regardless, I've now placed his feeder dish (small ceramic plate with a very shallow dip, no lip that would touch whiskers) upon an overturned shallow double feeder bowl i used to use with foster kittens which lifted his plate about 1 inch off the floor. Cannot deny that he seems to be eating a tiny bit more off the plate now!
