I did a bit of searching online for you... Apparently this issue can cause a rupture in the gallbladder and result in bile leaking into the abdomen if not treated. I would suggest going on the advice of your vets and having the surgery.

@Winchester when I had gallbladdersurgery - quite literally decades ago - it was major abdominal surgery. These days it is done laparoscopically. No idea how it is done for dogs. The anaesthesia would likely be the major concern.
