My cat has been having diarrhea for quite a while but now it's worse. She keeps going to the litter box and just squirting it out. Very foul smell. I'm thinking it's IBD. She's been to the vet many times and all test were ok. She's now on predisolone for almost 2 weeks but it's not working. I'm looking for something to give her a little relief. She can't take flagyl.

What food is she eating? What foods have you tried so far?
If she's not allergic to chicken, A bland diet of plain boiled chicken and rice for a couple of days can help settle an upset tummy. But this is only a short term solution as it is not a balanced diet far cats.
You could also incorporate a teaspoon of plan pumpkin puree into some wet food. Pumpkin is a safe fiber for cats that can help firm up stools. But she needs to have adequate water as well to ensure that it does not constipate her instead.

What tests did the vet do so far? Has a biopsy been done to determine IBD?
You could also try feeding her a LID diet or a diet consisting of a "different" proteins such as duck, venison, rabbit etc...
Other wills be able to give you some advice on that. I'm sure they will be around shortly.

Hi. In addition to what Jem Jem has offered, did the vet do a full fecal PCR which looks for bacteria as well as parasites? If not, you can take a sample in yourself (litter and all), if your vet accepts it that way, and have them do this test. Just bag it and refrigerate it up to 24 hours if you can't get it in to the vet's right away. The foul smell/diarrhea could be from bacteria - but, certain foods can cause that as well.

Did the vet also run a Chemistry Profile to look for metabolic issues, some of which can also cause diarrhea?

A bulk-forming laxative such as Metamucil (use the plain kind) seems to have a normalizing effect on cats with diarrhea. Also, a cat with diarrhea is losing key nutrients (electrolytes) that are essential for nerve function. To replenish them, you can give your cat unflavored Pedialyte. Mix with some water, or give with a dropper.

She's on a good diet of Koha duck and pumpkin. All blood tests have been done and also the stool, all negative.

How much pedialyte for a small cat? And maybe add some metamucil to the water too.

I'm not quite sure how much metamucil and pedialyte, hopefully some one else will answer.

BUT, I have herd that sometimes duck can be too rich for some cats. Also, does the food contain any peas? That can cause tummy issues in kitties too.

I have heard of about 1 tsp of Metamucil for cats - if she is small, maybe try half of that at first? It can be added to food or water.

For Pedialyte - you can first check your cat by gently pinching or pulling a small amount of skin on the upper back, right below the head/neck area. If the skin springs right back into place when you let go, that means she has enough fluids and won’t need any Pedialyte. If it is slow to go back into place, then the slower, the more dehydrated. If she does need it, you can try 3ml of unflavored Pedialyte per pound of body weight, no more than 3 times a day, and it doesn't have to be administered all at once.

Edit: Jem Jem may be onto something about the duck!

If fit's IBD or other gastrointestinal issue, a combo of diet and meds usually works but it may take awhile. The meds may vary for each cat. My cat was on pred, Cerenia, antibiotics, antacids, and a few other things when he was diagnosed with IBD.

If your vet doesn't know the cause of your cat's issues, it would be best to get another opinion from a different vet. Bring copies of the most recent blood work and tests so the new vet doesn't have to re-do them.

IBD is diagnosed with a biopsy although many people just opt for the lesser expensive ultrasound.

Instead of the chicken and rice diet as suggested as temporary diet, try a bland-ish novel protein food instead. Rawz is good but is pricey. There are other suggestions here: Likely Ibs, Unsure Of What To Do

My vet recommended Metamucil powder, mix it with her food. Of course she won't eat it. Any other recommendations for a liquid treatment for diarrhea? I see Nutri Vet has a diarrhea formula.

My vet gave one of my cats Rx Clay made my Rx Vitamins for Pets. It is hydrated calcium aluminosilicate clay. It is a very fine green powder that you mix with food. I only used it once as the cat it was prescribed for was dxed with an underlying issue that we treated. Like anything else, they have to eat the food that it is mixed in. My vet said that she uses it in cases where people say that if the cat doesn't stop having diarrhea that they will put it down (which I did not say and I am not suggesting that you did either). It is not a solution to the problem but a binder.

ProPectalin. It also contains a pro-biotic. Always worked for me and quickly, within a day, two at the most. My vet gave it to me the first time my cat had an IBD flare with liquidy stool. You can get it online without an rx. Not cheap but it works. It's not a cure however. You still have to find the underlying issue.

What types of tests were done? A fecal PCR test is best since that can rule out parasites such as Tritrichamonas feotus. That can not be diagnosed in house at the vets since it requires a specialized DNA test such as the PCR. One of the symptoms of Tf is diarrhea that is extremely foul smelling.

Hey, my cat Yoshi had diarrhea problems and used the royal canin gasto intestinal. A combination of dry / wet. It's one of the ones we usually recommend at work, and I've always had good feedback, so I tried it with my boy. He seems to love it also. Check out the pic of him eagerly waiting next to his dry food box! lol

My vet recommended Metamucil but my cat wouldn't take it with food. Someone mentioned Benefiber so I tried that in water. I used 1/3 teaspoon and my cat didn't respond well. She wouldn't eat for the rest of the day and was very lethargic. She also threw up once. Next day she was ok. I wonder if 1/3 tsp was too much or was it just the Benefiber?

If you're still having diarrhea problems. I would add saccharomyces boullardi to her wet food. I also have a cat who cannot tolerate a totally grain free diet and starts having diarrhea problems. We give him Royal Canin Gastrointestinal HE (dry) and Science Diet i/d (wet) mixed with the grain free food we feed everyone else and this keeps his poops in check.

Try Endosorb. We used it for Keith while we were trying to find out what was causing the diarrhea. (Turned out to be a food allergy) It firms up the poop and also is supposed to help with pain and cramps associated with diarrhea. It was inexpensive (ordered it from a pet medication website, around 20 cents per tablet), worked quickly (saw improvement within 2 days), and he had no problem eating it mixed in his food. It's a hard chalky tablet that can easily be crushed into a powder that dissolves in wet food and is tasteless. I think we gave him half a tablet/twice a day, but he is a big boy.
It's not a cure, but it greatly relieves the symptoms until you can figure out the underlying condition.

Hi. My cat has been on grain free wet cat food and was doing great on it until two months ago. I've been giving her the same food mixed with cat enzymes that has acidophilus in it which has made her like a kitten, firmed her stools and increased her appetite. But now, she's acting like she's got an UTi, although the vet says her urine looks perfect (no crystals/stones, signs of blockage etc)
Can you tell me why a grain free diet would cause diarrhea?
