It looks more like scabies to me. Has he been scratching at it? You should take him to the vet ASAP... both of those conditions are serious and need to be addressed immediately.

For one cat, just once a month. Because it clumps really well, I don't have to change out the litter when a clump falls apart.

I recommend the Weruva Hinoki Wood and Green Tea litter. It clumps really well, it is very low in dust, and the pellets are large enough to not track. Sure, you will find one outside of the litter box every now and then, but that it much better than any clay litter I’ve used.
