Pupils were different size and now one eye is cloudy?

Hello all

I just found this forum after googling but unfortunately there hasn’t been an update from user L lynxx (link below to their thread)

Our York chocolate Storm is almost 10 months old and hasn’t had any health issues up until yesterday.

We were out for 2 hours and when we came back we noticed that she keeps her left eye closed but without discharge. She was eating but then deteriorated rather quickly and was quite lethargic.

We video called a vet and described that we don’t know about any injury or such but that she kept her eye closed, is rather sleepy and now her pupils are different sizes.

We also saw the pupil of her left eye is cloudy and not black like it is in her normal eye. Also when she’s in a dark room looking into a light room the reflection of the effected eye is yellowish?

This was her on Friday with normal eyes:

The vet on the phone recommended seeing someone in person right away. We went to a 24h vet as it was Saturday evening, they did a dye test and looked at the eye for any scratches etc, and said they didn’t see the reflection difference and we were told that she might’ve scratched herself with her claws while cleaning and that the third eyelid has some sort of inflammation. We were given eye drops and were asked to see the vet again (our normal one) on Monday, what bugs me is that I can see it’s not only the third eyelid but also her pupil being cloudy and reflecting light strangely in the effected eye.

Does anyone here have any information about this?

As she’s only 10 months old and was always healthy we have no idea what it could be and now it’s Sunday, vets are closed, and our minds are racing because the vet did not address the issue with the pupil at all, might’ve overlooked it?

Hi. How is your cat doing now? Are you using the drops the ER vet gave you? I almost think it looks like there is a bit of an improvement since the previous pics - do you agree?

If she is not in any pain and eating/drinking fine, keep up with the drops and then let your regular vet see her as planned. Keep the pics so you can show them to your vet to see if he thinks there is some improvement - it might help him to assess the issue. Given your cat's age, and how quickly this came about, it would seem that it is likely from some sort of injury.

When pupils are a different size and cloudiness occurs, it usually means an infection, from my experiences anyway. It could also mean she fell and struck her head. I would draw blood and check for infection somewhere else in her body, or any other possible causes. An injection of a steroid may help too. Otherwise, it may be an eye infection but the drops should be helping. Though they take a few days to start working. I had a cat with the same symptoms and they cleared up with an oral antibiotic, which I give in a lickable treat.

I'm sorry for your kitty.
From my limited experience, a cloudy eye and/or other ocular issues, lethargy, lack of appetite, are often related to FIP... Something that nobody would ever like to hear.
So, my advice is to take your kitty to your vet and have a blood test done with an electrophoresis test as well to assess the level of globulines, in order to rule out this scary disease.

Please let us know. Meanwhile our fingers are crossed! 🤞

My girl, Rocket, had uneven pupils a few years back:

She did get eye drops within 12 hours of the pupils becoming uneven so I don't know if she never would have gotten cloudiness or if I caught it early.

My girl is positive for Feline coronavirus, so FIP was a possibility. Along with dozens of other conditions. She had a few blood draws for testing along with xrays and they were talking a MRI if it didn't improve with the medicine. Her eye started to self resolve after about a week and she was back to normal by the time we saw the neurologist. They determines that she had bumped her head hard enough to cause bleeding in the eye which caused a blood clot blocking light in the impacted eye.

Just follow the doctor directions and hopefully it is one of those weird one off minor scares.

So what we’ve done now is a follow up with our normal vet today (as discussed with the 24h vet on Saturday). They did another dye test and could not find anything in her eye itself like scratches or etc. but did see the difference in her pupil dilation and did a pressure test and determined that the pressure is different in the eyes and higher in her healthy eye. ☹
We were given anti inflammatory suspension to give to her food and ordered to continue the eye drops. I felt like her eye cloudiness has improved since Saturday but the pupils are still dilating differently. They didn’t seem to be concerned about FIP and didn’t take blood etc. but I’ll definitely double check this on Wednesday on our next appointment. She’s eating more now since Saturday too which I think is a good sign and she’s just very cuddly and probably enjoys all the extra attention being carried everywhere haha, like she wasn’t already before

We still have no idea what could’ve been the cause and the vet didn’t have any idea either

It's probably a good sign that the IOP in the affected eye is lower, that suggests it's unlikely to be glaucoma. Did they measure her blood pressure too? If they didn't do bloodwork the vets probably don't think an eye infection is likely and probably didn't note hyphema; this is consistent with their not finding external trauma with the fluorescein and the fact that her eye is dilated rather than miotic.

Hopefully she is kept indoors only? for her own safety and health and your peace of mind.
This condition can be caused by many very different things. I would want bloodwork done right away.

I'd definitely want blood work, but I also like having blood work history so we know both what is normal and how they typically respond. My regular vet was closed when Rockets eye did that so we had to see the 24 hour specialist, probably why they ran every test known to cat right off the bat. Also why she saw a neurologist, cardiologist and some other ist fairly quickly.

We went back to the vet today, her right eye pressure went down a little from 30 to 23, but they still want to check it again next week. All good this morning until now. Seems like the same thing is happening to her eye like it did with the one on Saturday? What can it be? We’re going to have to go back to the vets tomorrow but we’re just so clueless because they all say they don’t have an answer either…

And yes, she’s indoors only, if she goes outside it’s on the leash and supervised and the last time she went out was weeks ago.

Could it be that the infection just went over to the other eye? Maybe we can treat it the same way like we did with the left one, which now looks perfectly normal again?
The vet was concerned about the pressure though so I’m not sure what’s going on. We are also thinking about changing the vet and are going to call someone tomorrow to see their availability and opinion on this.

Very mysterious. I hope you're not in the US because if you are, tomorrow is a holiday for them and I don't know if you'll find any vet open. *PRAYERS* continue for your little beauty!

Oh dear, I didn't realize "higher in the healthy
eye" meant the IOP was higher than normal there. And if the left eye IOP was actually at "normal" or above-normal levels then that's more serious and could point to glaucoma, since generally in cases of inflammation the IOP should be lower than normal. It's worrisome that both eyes seem to now be affected, and I hope the vets are exploring glaucoma and systemic hypertension as potential causes.​

Another update 😫
We changed the vet. The left eye has now completely healed so we are almost sure it’s conjunctivitis or uveitis not glaucoma or virus related and it can be treated with the same eyedrops. I didn’t know you have to treat both eyes to avoid it from spreading to the other eye! Nobody told us! 🙃

We are now more confident in our new vet as they seemed way more competent and more human AND actually gave us options of possible causes and diagnoses (conjunctivitis or uveitis which can be healed with medication/ eye drops and can be caused by a small scratch to the eye) and a simple answer to what could’ve caused a higher eye pressure which went down again: infection.

Fingers crossed the eye drops work their magic on her right eye too and a follow up is scheduled on Monday after cat mamas birthday, so hopefully the best gift: a healthy Storm 🥰

We hope you have happy holidays for those of you who celebrate this week and thank you a million for your suggestions and well wishes!

Hopeful update, thank you, Storm26 Storm26 ! I've had uveitis and there was no treating the other eye, only the affected one. I don't have TikTok or the time for yet another site, but I am very glad to hear this news and I hope you'll keep us informed of her wellbeing!

I'm so glad your new vet feels the cause is a minor infection, and that the left eye is almost healed! Generally with at least acute, uncomplicated anterior uveitis the pupil becomes constricted rather than dilated, while dilation typically indicates uveitis with secondary glaucoma, so your first vets may not have considered infection as likely. If it is uveitis, though (did they do a slit beam to check for aqueous flare?), it's extremely important to identify and treat the cause promptly and continue monitoring IOP as it can easily lead to glaucoma. Here's hoping it's "just" keratitis or conjunctivitis!
