Swollen private parts/swollen stomach? Help!

About five years ago, I got two kittens, about 3 months old. They ran away and one came back, but the other didn't. Now, five years later, I think that same cat has come back. He/she has been extremely aggressive to my other cats, but surprisingly friendly to me. He/she seems to recognize the house and seems quite comfortable exploring the house. Here's the problem: his/her private parts look like a male cats', but his/her stomach is very swollen and hard and it really seems like he/she's pregnant. It's definitely not fat, because he/she is very bony (unhealthily so) on the rest of his/her body. She's been around for a month and now has a healthy small amount of fat. I think his/her stomach has swollen more in the month that he/she's been around. So normally I would conclude that she's pregnant, except his/her private parts look like a male cat's. He/she weighs 14.2 pounds, but is a petite cat and still on the thin side. I will try to get pictures soon. Any advice/thoughts on this will be very appreciated! Feel free to ask me any more questions if you need more information.

Could be worms. Could be sick. A vet visit is needed.

Do you intend to keep this kitty? Do you have any other cats? If so, it's probably best to keep them separated until you get him to the vet to make sure he's not contagious.

Do you intend to keep this kitty? Do you have any other cats? If so, it's probably best to keep them separated until you get him to the vet to make sure he's not contagious.
I have a lot of other cats, but they are outside cats and they come and go as they please. The cat in question does not interact with other cats and is very hostile towards the other cats.

Keep us posted, and do try to get the cat to a vet. This could be a simple fix, or something requiring more drastic measures. I do not believe that it will resolve on its own, however.

Is it possible that the cat is actually pregnant? I think it's possible that I'm wrong about him being a boy.

It is possible, and if the cat really is female, and not spayed, almost certain. If you can get a look at the tummy and can clearly see nipples, that would certainly indicate that you have a new family on the way. Most cats nipples are usually covered by their fur, but in late pregnancy, you can see them more clearly, as below:
